These simple, quick and confidential At-Home Kits will allow you to obtain your:

  • Total Cholesterol
  • Triglycerides
  • HDL Cholesterol (or good cholesterol)
  • LDL Cholesterol (or bad cholesterol)
  • Glucose (A1c)

Know your numbers and you can take action to reduce your risk of developing serious health problems like heart disease, diabetes and other chronic illnesses.

All it takes are six easy steps:

Just log on to your Live Well web portal, www.LiveWellatWCIF.net }

Click on the At-Home Biometric Kits link on the Bulletin Board.  Follow the prompts to request your kit.

When your kit arrives, follow the easy instructions it contains.  Click here to watch a video tutorial }

Mail your kit back in the provided envelope, and you will receive an email when your results are ready.
 

What's Next?

Use your results to complete your Wellness Assessment and earn 5 Live Well wellness credits.
Then, claim 2 more credits for completing a biometric screening.

You just earned 7 credits!  All you need are 10 credits total to earn $125 cash!

Important: Please request a kit only if you will complete and return it.
