Kits are
free through the Live Well at WCIF
program for a limited time!
These simple, quick, confidential kits will
measure your:
- Total Cholesterol
- Triglycerides
- HDL Cholesterol (or good cholesterol)
- LDL Cholesterol (or bad cholesterol)
- Glucose (A1c)

If you know your values for each item listed above you can take action to
reduce your risk of developing serious health problems like
heart disease, diabetes and other chronic illnesses.

It just takes 6 easy steps to complete your At-Home Kit:
Start by logging on to your Live Well web portal,
www.LiveWellatWCIF.net

Click on the Health Screenings link on the
left-hand menu. Follow the prompts to request your kit.
When your kit arrives, follow the easy instructions it
contains.

Mail your kit back in the provided
envelope, and you will receive an email when your results are
ready.
What's Next?
Use your results to complete your Wellness Assessment and
earn 5 Live Well wellness credits.
Then claim 2 more credits for completing your biometric screening.

All you need are 10 Wellness Credits before November 15 to earn $125 cash!*

In order to increase the health and welfare of its
members, WCIF/WCIP provides this service at no
additional cost to participants. However, WCIF/WCIP
urges members to request a kit only if they will
complete and return it. WCIF/WCIP reserves the right to
bill members for any unreturned kits. The cost per kit
is approximately $65. |

Members must be active on the program end date of November
15th in order to qualify. Wellness
