Enter your employee ID (SSN) under
First Time Here? and select your affiliation.

Click Enter Site. Enter your gender, date of birth and employee ID
once again. Then click Next. |

Fill in the rest, accept the Terms of Use and
click Submit. |

medically insured spouses/domestic partners. Wellness Assessment completion is mandatory for incentive
qualification. Members must be active on the program end date of November 15, 2010, in order to
qualify.
Incentives will be mailed to qualifying members within 45 days of the program end date.
